About this form

Design of plant which is deemed to be high risk must be registered with WorkSafe prior to use in the workplace. To have your plant design registered with WorkSafe, you will need to complete this form and lodge it together with relevant design drawings of the plant. The application form must be signed by the Design Notifier, Designer and the Design Verifier and must make reference to the Design Standards used to design the item of plant. A registration fee (GST exempt) is payable.
