WorkSafe Victoria Provider Registration Requirements
Service providers can register with WorkSafe Victoria (WorkSafe) to provide services to workers under Victorian workers compensation legislation.
A provider of services to workers must satisfy the relevant provider eligibility requirements listed below by service type. Please use the following information for your service type to assist in completing the WorkSafe Application for Registration to Provide Services to Workers form where required.
Updating your details
For service providers who are already registered with WorkSafe who need to update their practice details (practice address, preferred mailing address, phone number, email) please advise WorkSafe in writing.
For service providers who are already registered with WorkSafe who wish to be paid via EFT or who need to update their EFT details, please complete the Electronic Funds Transfer Application Form

Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Acupuncture: Chinese Medicine Practitioners
This section applies to Chinese medicine practitioners registered in the Division of Acupuncturists under the Health Practitioner Regulation National Law.
Chinese medicine practitioners must complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Chinese Medicine Board of Australia registration number in the space provided.
A WorkSafe provider number will be issued for invoicing purposes.
Chinese medicine practitioners must maintain at all times a minimum of $5million professional indemnity insurance coverage.
Attendant Care
All applicants must complete the WorkSafe Application for Registration to Provide Services to Workers form and provide all requested documentation including copies of qualifications and membership (or evidence of eligibility for membership) where required. WorkSafe will contact providers if additional information or clarification is required before processing the application.
Attendant Care
Attendant care providers must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form.
- provide documented current proof that:
- they have substantial experience in attendant care or a related field.
- they are committed to operating within a structured program. Information such as organisational philosophy of the provider, previous performance, links with employers, government agencies, hospitals, health practitioners, and references will be considered.
- they have the resources to provide quality service delivery at all locations in which they intend to operate. This includes access and exit procedures and reliable contact and back-up to cater for worker attendant care needs.
- an adequate supervisory structure exists for service provision, case review and staff guidance and regular and relevant staff training. Training may be provided by a health professional contracted by the agency.
- supervisors are qualified rehabilitation professionals with formal qualifications in a medical, health or related area. This includes a diploma, degree or graduate diploma in medicine, health sciences, behavioural sciences, social work or rehabilitation counseling or nursing qualifications.
- they have documented guidelines, protocols and practices for ensuring the health and safety of their employees.
- employees demonstrate an understanding of the legal and human rights of workers, the maintenance of appropriate carer/worker relationships and duty of care issues.
- any person who is engaged in providing attendant care to a worker is suitably trained by a qualified rehabilitation professional in providing attendant care services. All attendant carers must possess a current first aid certificate. Attendant carers who possess the Personal Carer Certificate Level 3 are preferred.
- provide written references from at least two referring medical practitioners, authorised agents or funding bodies who have utilised services provided by the applicant and who can attest to these services.
- provide written confirmation of a minimum of $5 million professional indemnity insurance coverage and a minimum of $5 million public liability insurance coverage with the initial application
- maintain at all times a minimum of $5 million professional indemnity insurance coverage and a minimum of $5 million public liability insurance coverage.
Registration can be maintained by the provider continuing to meet the registration criteria and any amendments to the criteria made by WorkSafe. WorkSafe reserves the right to audit providers to ensure compliance with all criteria and standards.
A WorkSafe provider number will be issued for invoicing purposes.
Chiropractic
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Chiropractic
Chiropractors must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Chiropractic Board of Australia registration number in the space provided
- maintain at all times a minimum of $10 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For chiropractors who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Dental
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Dental
Dentists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Dental Board of Australia registration number in the space provided
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
The dentist’s Medicare number(s) will be used by WorkSafe for invoicing purposes.
Dietician
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Dietician
Dieticians must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide proof that they are an Accredited Practicing Dietician if they are not registered with Medicare
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For dieticians who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Exercise Physiology
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Exercise Physiology
Exercise physiologists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide written confirmation from Exercise and Sports Science Australia that they have been approved as a member with accreditation in exercise physiology if they are not registered with Medicare
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For exercise physiologists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Hearing Services
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Hearing Services
The following criteria will be used in the process for selection of providers for registration by WorkSafe to supply and fit WorkSafe approved hearing devices.
Requirements – qualifications and experience:
Applicants must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide a copy of qualifications as follows:
- Audiologists: The Master of Clinical Audiology (MCAud)
- Audiometrists: Certificate IV in Audiometry (end 2008); Diploma of Hearing Device Prescription and Evaluation AND Certificate IV in Audiometric Assessment (post 2008)
- provide proof of membership or eligibility for membership of the:
- Audiological Society of Australia (ASA), AND/OR
- Australian College of Audiology (ACAud) with Certificate of Competency in Diagnostic Assessment and Hearing Aid Dispensing desirable.
Requirements – quality of service
Applicants must provide documented current proof that facilities and equipment meet the required standards and that equipment is fully maintained and calibrated:
- Ambient background certificates compliant with the AS ISO 8253:2009 – Acoustics – Audiometric test methods standards (as amended or replaced from time) or AS/NZS 2169.4:2005 Maximum Acceptable Noise Levels for each nominated practice location. Please provide as an attachment to your application information identifying which certificate corresponds to each practice location.
- Equipment Calibration certificates compliant with the ASIEC 60645.1:2002 Electroacoustics – Audiological equipment (as amended or replaced from time) and obtained every 12 months for equipment used. Please provide as an attachment to your application information identifying which equipment is used at each practice location and the corresponding calibration certificates.
By signing the Provider Acknowledgement on the WorkSafe Application for Registration to Provide Services to Workers form, the applicant agrees to provide quality services including:
- commitment to high ethical standards in clinical practice, demonstrating a knowledge and commitment to the Code of Practice and professional service standards issued by the ASA or ACAud.
- agreement to participate in continuing education relating to the assessment of hearing loss and the fitting of hearing aids and hearing devices approved under Victorian workers compensation legislation.
- agreement to participate in service review processes in relation to all aspects of the conduct of hearing devices and assessment of hearing loss.
- agreement to participate in WorkSafe complaints investigation process.
- a demonstrated commitment to provide high quality services to injured workers.
Requirements – business practice
The applicant must:
- provide written confirmation that they have no arrangement with any referring person or entity whereby the referring person or entity gains any financial benefit from a referral to the applicant of a worker, as well as a declaration of any other conflict of interest
- provide details of directors (those with ultimate financial responsibility for the organisation) and subsidiary companies
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
Once approved, a WorkSafe provider number will be issued for invoicing purposes.
Household Help (Gardening and/or Domestic Cleaning)
All applicants must complete the WorkSafe Application for Registration to Provide Services to Workers form and provide all requested documentation including copies of qualifications and membership (or evidence of eligibility for membership) where required. WorkSafe will contact providers if additional information or clarification is required before processing the application.
Household Help (Gardening and/or Domestic Cleaning)
Application for registration to provide household help services may be made by individuals or organisations.
Household help providers must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- sign the Provider Consent and Declaration on the WorkSafe Application for Registration to Provide Services to Workers form to provide acknowledgement that they have appropriate experience to satisfy industry requirements.
- provide written confirmation of a minimum of $5 million public liability insurance coverage with the initial application
- maintain at all times a minimum of $5 million public liability insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Loss and Grief Counselling
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Loss and Grief Counselling
Loss and grief counsellors must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide written confirmation that they are an accredited counsellor with the National Association of Loss and Grief (Victoria),
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Medical
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Medical
WorkSafe has adopted the Medicare Australia provider numbers and registration details for medical practitioners.
Medical practitioners must maintain at all times a minimum of $1 million professional indemnity insurance coverage.
The medical practitioner’s Medicare number(s) will be used by WorkSafe for invoicing purposes.
Naturopathy
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Naturopathy
Naturopaths must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide written confirmation that:

- Australian Natural Therapists Association Ltd (Naturopath)
- Australian Natural Practitioners Association Inc, OR
- Complementary Medicine Association
OR
- their qualifications entitle them to full membership of the:
- Australian Natural Therapists Association Ltd (Naturopath), OR
- Complementary Medicine Association

they are a full member of the:
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Nursing
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Nursing
Application for registration to provide nursing services may be made by individuals or organisations.
Individual nurses must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Nursing Board of Australia registration number in the space provided
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
Organisations must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include the Nursing Board of Australia registration number of nurses who will be providing nursing services to workers
- maintain at all times a minimum of $1million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Occupational Therapy
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Occupational Therapy
Occupational therapists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Occupational Therapy Board of Australia registration number in the space provided
- maintain at all times a minimum of $1million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For occupational therapists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Optometry
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Optometry
Optometrists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Optometry Board of Australia registration number in the space provided
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For optometrists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Osteopathy
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Osteopathy
Osteopaths must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Osteopathy Board of Australia registration number in the space provided
- maintain at all times a minimum of $20 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For osteopaths who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Physiotherapy
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Physiotherapy
Physiotherapists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Physiotherapy Board of Australia registration number in the space provided
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For physiotherapists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Podiatry
Health Practitioner Regulation National Law Registered Providers

Podiatrists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Podiatry Board of Australia registration number in the space provided
- maintain at all times a minimum of $5 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For podiatrists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Professional Interpreting Services
All applicants must complete the WorkSafe Application for Registration to Provide Services to Workers form and provide all requested documentation including copies of qualifications and membership (or evidence of eligibility for membership) where required. WorkSafe will contact providers if additional information or clarification is required before processing the application.
Professional Interpreting Services
Application for registration to provide interpreting services may be made by individuals or organisations.
Professional interpreting service providers must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide written confirmation that they have:
- National Accreditation Authority for Translators and Interpreters (NAATI) Professional level accreditation (formerly referred to as Level 3), or
- NAATI Paraprofessional level accreditation (formerly Level 2) for languages where Professional level accreditation is not available for assessment, or
- been assessed as suitable to provide interpreter services based on education and experience when NAATI accreditation at any level is not available
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Prostheses and Orthoses
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Prostheses and Orthoses
Prosthetists and orthotists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide a copy of:
- written confirmation of their eligibility to be a member of the Australian Orthotic Prosthetic Association Inc. (CPO-AOPA only), OR
- their tertiary qualifications of Bachelor of Prosthetics and Orthotics
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Psychology
Health Practitioner Regulation National Law Registered Providers
The mandatory requirements for registered practitioners are governed by the Australian Health Practitioners Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme. Providers must at all times maintain Board registration in order to be a WorkSafe registered provider.
Psychology
Psychologists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form and include their Psychology Board of Australia registration number in the space provided
- maintain at all times a minimum of $2 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For psychologists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
Remedial Exercise (Gym and Swimming Programs)
All applicants must complete the WorkSafe Application for Registration to Provide Services to Workers form and provide all requested documentation including copies of qualifications and membership (or evidence of eligibility for membership) where required. WorkSafe will contact providers if additional information or clarification is required before processing the application.
Remedial Exercise (Gym and Swimming Programs)
Remedial exercise service providers must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- sign the Provider Consent and Declaration on the WorkSafe Application for Registration to Provide Services to Workers form to provide acknowledgement that they have a duly qualified person as determined by WorkSafe to supervise programs as follows:
Commercial gymnasiums/pools
- TAFE Certificate III in Fitness or equivalent, OR
- Exercise-related tertiary qualification or equivalent, for example:
- Bachelor of Human Movement
- Bachelor of Exercise Science
- Bachelor of Sport Science.
Allied health practice or hospital based gymnasium and/or pool
- TAFE Certificate III in Fitness or equivalent, OR
- Suitable training and experience in exercise prescription and progression, e.g. exercise physiologist, physiotherapist, chiropractor or osteopath or an allied health assistant or a student on placement under the supervision of a suitably trained and experienced allied health professional.
- provide written confirmation of a minimum of $5 million public liability insurance coverage with the initial application
- maintain at all times a minimum of $5 million public liability insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes.
Remedial Massage
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.
To support your application you may be required to provide evidence such as relevant tertiary qualifications, professional experience or membership of a professional association (or evidence of eligibility for membership). WorkSafe will contact you if additional information or clarification is required before processing the application.
Remedial Massage
Remedial massage providers must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- provide written confirmation:
- that they are a member of the:
- Australian Association of Massage Therapists Ltd. (AAMT)
- Association of Massage Therapists (NSW) Ltd
- Australian Natural Therapists Association Ltd (ANTA)
- Australian Traditional Medicine Society Ltd, OR
- Institute of Registered Myotherapists (IRMA)
OR
- that their qualifications entitle them to full membership of the Australian Natural Therapists Association Ltd (ANTA)
- that they are a member of the:
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.

Speech Pathology
Allied Health Non Board Registered Providers
For non-board registered allied health providers, the qualifications of the service provider, business registration and insurance coverage must be acceptable to WorkSafe.

Speech Pathology
Speech pathologists must:
- complete the WorkSafe Application for Registration to Provide Services to Workers form
- if they are not registered with Medicare, provide a copy of:
- written confirmation of their eligibility to be a member of Speech Pathology Australia,
OR - their tertiary qualifications: Bachelor of Speech Pathology, Bachelor of Applied Science (Speech Pathology), Bachelor of Health Sciences (Speech Pathology) or Master of Speech Pathology
- written confirmation of their eligibility to be a member of Speech Pathology Australia,
- maintain at all times a minimum of $1 million professional indemnity insurance coverage.
A WorkSafe provider number will be issued for invoicing purposes. For speech pathologists who are registered with Medicare, their Medicare number(s) will be used as the provider number.
