Apply for WorkSafe insurance

Your application for a WorkSafe insurance policy must be received within 60 days of the date you meet the criteria of requiring a policy.  The premium amount you pay will be reviewed each year, or if your circumstances change.  

To take out a WorkSafe insurance policy, you will need to fill out either the PDF or online application form below and send it to the WorkSafe agent of your choice.
