About this form
Theatrical fireworks may be discharged for theatrical, entertainment or similar purposes. A single occasion Theatrical Fireworks licence is issued for a single date and venue. You must lodge your licence application at least 21 days prior to the fireworks event. You must be at least 18 years of age to apply for this licence or to discharge fireworks. A fee of $120 (No GST applies) is payable for the issue of this licence. WorkSafe now requires proof of identification to be provided with this form. Therefore unless you have previously done so, you will need to complete an Identification Form - Natural Person or Identification Form - Non-Individual and submit it with your application.
