About this form
The design of plant listed in Schedule 2 of the Occupational Health and Safety Regulations 2017 must be registered with WorkSafe. To apply to have your plant design registered with WorkSafe, you will need to complete this form.
The design of prescribed equipment listed in Schedule 1 of the Equipment (Public Safety) Regulations 2017 must be notified to WorkSafe. You will also need to complete this form to give notice of your prescribed equipment design.
WorkSafe administers the two schemes in the same way and an application for plant design registration also meets the requirements of having provided notice of prescribed equipment design. The completed form must be lodged with WorkSafe together with relevant design drawings of the plant/prescribed equipment and include a design verification statement. The application form must be signed by the person registering/notifying the design and the design verifier and must make reference to the published technical standards or engineering principles used to design the plan/prescribed equipment.
A registration fee of 20 fee units is payable. The current registration fee is $284.40
