Employer electronic funds transfer (EFT) application forms

Employers can use these forms to request EFT refunds.

Employer EFT form

All employers can use the ‘employer’ form to request to receive claims reimbursements and/or premium refunds by EFT.

Workplace EFT form

Employers with multiple workplaces can use the ‘workplace’ form to request to receive EFT claims reimbursements into different workplace accounts. You will need to complete a separate form for each workplace.
