Competent inspection statement – item of plant

What it contains

Use this form to confirm that an amusement structure item of plant has been both:

• inspected by a competent person
• assessed by that person as being safe to operate.

A ‘competent person’ means a person who has:

1. educational or vocational qualifications in an engineering discipline relevant to the part of the plant to be inspected, and
2. knowledge of the technical standards relevant to the part of the plant to be inspected.

Who must complete the form

This form must be completed by the competent person inspecting the amusement structure.

If the amusement structure contains an electrical installation, the installation needs to be inspected by either:

• a person qualified to inspect electrical installations
• a person assisted by the qualified person

Submission requirements

This form needs to be submitted as part of the item of plant registration application.

The date of the inspection must be within 12 months of the application date.
