Design verification statement (for amusement structures designed before 1 July 1995)

What it contains

A PDF form to confirm plant design verification to WorkSafe. This form only applies to amusement structures designed before 1 July 1995. WorkSafe Victoria need verification that the amusement structure plant design is safe and was produced in line with the published technical standards and engineering principles specified by the designer.

This form must be completed by the design verifier.

The design verifier must:

• have the appropriate skills and knowledge, gained through relevant qualifications or experience, to verify the plant design
• not have participated in the plant's design
• reside in Australia.

Other plant designs

To confirm plant design verification for all other plant designs, including amusement structures designed after 1 July 1995, you'll need to complete the Design verification declaration.
