Fill out this form to request RTO CI Administrator access to your RTO's myWorkSafe account to create, manage and pay for CI applications.

This form must be completed by an authorised person for the RTO, as listed on training.gov.au. Before you start filling out the form, make sure each new RTO CI administrator has their own myWorkSafe account.
