What it contains

Spreadsheet template instructions:

1. Review pre-filled information in the 'Self-insurer details' table and amend as required. Complete any remaining gaps in the table.
2. Complete the 'Self-insurer and subsidiary details' table after:
   • populating the 'Subsidiary sector & key info' tab
   • populating the 'subsidiary hazard profile' tab
   • collating Subsidiary safety performance indicators. Submit the completed SPIs to WorkSafe together with this form.
3. Complete the 'Third-party audit details' table for subsidiaries certified to ISO 45001.
   • Submit relevant documentation to WorkSafe together with this form.
4. Submit all information to [email protected].
