Applications

All applicants must complete the WorkSafe Application for Registration to Provide Services to Workers form and provide all requested documentation including copies of qualifications and membership (or evidence of eligibility for membership) where required. WorkSafe will contact providers if additional information or clarification is required before processing the application.

Remedial exercise (gym and swimming programs)

Remedial exercise service providers must:

  • complete the WorkSafe Application for Registration to Provide Services to Workers form
  • sign the Provider Consent and Declaration on the WorkSafe Application for Registration to Provide Services to Workers form to provide acknowledgement that they have a duly qualified person as determined by WorkSafe to supervise programs as follows:
    • Commercial gymnasium/pools
      • TAFE Certificate III in Fitness or equivalent, or
      • Exercise-related tertiary qualification or equivalent, for example:
        • Bachelor of Human Movement
        • Bachelor of Exercise Science
        • Bachelor of Sport Science.
    • Allied health practice or hospital based gymnasium and/or pool
      • TAFE Certificate III in Fitness or equivalent, or
      • Suitable training and experience in exercise prescription and progression, e.g. exercise physiologist, physiotherapist, chiropractor or osteopath or an allied health assistant or a student on placement under the supervision of a suitably trained and experienced allied health professional.
  • provide written confirmation of a minimum of $5 million public liability insurance coverage with the initial application
  • maintain at all times a minimum of $5 million public liability insurance coverage.

A WorkSafe provider number will be issued for invoicing purposes.
