Application to conduct a national police check and ASIO security assessment

What it contains

Complete this form if you wish to consent to:

• A national police check for the purpose of WorkSafe Victoria (WorkSafe) obtaining known information concerning you pursuant to the Dangerous Goods Act 1985 and Regulations to determine whether you satisfy the requirements to hold a Licence or Permit; and/or
• An ASIO security assessment for the purpose of obtaining a Dangerous Goods Licence or Permit with WorkSafe pursuant to the Dangerous Goods Act 1985 and Regulations (ASIO only requires consent once).
