Design verification declaration

What it contains

A PDF form to confirm plant design verification to WorkSafe.

WorkSafe Victoria need verification that the plant design is safe and was produced in line with the published technical standards and engineering principles specified by the designer.

This form must be completed by the design verifier.

The design verifier must:

• have the appropriate skills and knowledge, gained through relevant qualifications or experience, to verify the plant design
• not have participated in the plant’s design.

Amusement structures designed before 1 July 1995

To confirm plant design verification for amusement structures designed before 1 July 1995, you’ll need to complete the Design verification statement (for amusement structures designed before 1 July 1995).
