Design verification declaration

What it contains

A PDF form to confirm plant or equipment design verification to WorkSafe.

WorkSafe Victoria needs verification that the plant or equipment design is safe and was produced in line with the published technical standards and engineering principles specified by the designer.

Design verifier

This form must be completed by the design verifier.

The design verifier must:

• have the appropriate skills and knowledge, gained through relevant qualifications or experience, to verify the plant design
• not have participated in the plant’s design.
