This fee schedule includes the variable payment rates for the following:

Services delivered at practitioners' rooms
Services delivered at a hospital
Services delivered at patient's home or work
Hydrotherapy
Orthosis
Telehealth services
Requests for information (ie Reports/Questionnaires)
