What it contains

This is the Support Coordination pilot's induction pack for an injured worker.

The pack contains a brief introduction into the pilot and the steps required for an injured worker to participate.

The injured worker is required to complete the consent form on page 3 of pack and must be returned to the workers Case Manager. This is a mandatory requirement.

The pack also contains a brief one page summary of each of the two service provider's participating in the pilot - Melbourne City Mission (MCM) and Star Health.
