What it contains

Support coordination is a service aimed at identifying and connecting injured workers with healthcare and community based services. A support coordinator will assist you and your treating team in coordinating services to help you achieve your individual goals.

Support coordinators have experience supporting individuals, families and treating healthcare providers to achieve recovery goals. Support coordinators are independent of your case manager and WorkSafe.

To participate in the program, please complete and return the consent form to your case manager. You can choose to complete this via post or email.
