Collection notice
WorkSafe Victoria (WorkSafe) handles personal and health information in accordance with the Privacy Act 1988 (Cth), Privacy and Data Protection Act 2014 (Vic), Health Records Act 2001 (Vic), and its Privacy policy.
WorkSafe collects personal information from this application to:
- assess the application and administer the licence
- administer and enforce legislation administered by WorkSafe.
WorkSafe may disclose your information to:
- emergency services
- law enforcement and security agencies, including Victoria Police
- its contractors and agents
- a court or tribunal
- regulatory agencies and authorities
- Registered Training Organisations
- any person to whom it relates, or is required by law, to obtain it.
WorkSafe may also disclose your licence status to employers, prospective employers or members of the public who wish to check this status. Information collected will not otherwise be shared with any other third parties unless required or authorised by law.
Collection of your personal and health information may be required by the Occupational Health and Safety Act 2004 or Dangerous Goods Act 1985 and associated regulations.
If you do not provide any or all of the information, your application may:
- not be accepted or processed
- experience delays due to the need for additional information.
