Ensure your form is completed correctly and all supporting information is provided.

You must provide additional supporting documentation required in sections 7 and 9.

You must also complete an application to conduct a National Police check and ASIO security assessment form including your drivers licence details.

You must provide:

• 100 points of identification: at least one with a photograph and signature.
• Identification showing your date of birth.

• If you are an individual, complete the natural person identification form.
• If you are a company or body corporate, complete the non-individual identification form.

If you have previously completed these identification forms, you do not need to complete them again, unless your details have changed.

You can complete your payment details on the form.

Complete the application form and also provide:

• Identification form (natural person or non-individual).
• 100 points of identification (at least one with your photograph and signature).
• Identification showing your date of birth.
• Application to conduct a National Police check and ASIO security assessment form including your drivers licence details.
• Additional supporting documentation required in sections 7 and 9.
• Required fee.

Send your completed application, supporting documents, and identification form to WorkSafe's licensing branch by mail or email.
