Shape

What it contains

The Person-Centred Plan (PCP) outlines individualised goals and actions for the injured worker who has consented to participate in the Support coordination program.

The support coordinator should complete the PCP once the injured worker has signed the program's consent form and supplementary information has been provided by the case manager.

The PCP is the key document for support coordinators to tailor an injured workers program to achieve recovery goals, return to work and/or community integration.

This form outlines the key accountabilities and options to achieve the injured worker's goals, some of which may require further discussion with the case manager if additional funding is needed.
